CHILD HEALTH AND DISABILITY PROGRAM

 

In order to assure optimum health of students and comply with Child Health and Disability Program (CHDP) state statute, the Board requires all students to have a health assessment no more than six months prior to entering kindergarten.

In order to enter the schools of this District, all kindergarten and first grade first time entrants must provide a certificate approved by the California State Department of Health or a qualified physician documenting that the child has received the appropriate health assessment.

A waiver signed by the child’s parents or legal guardian indicating that they do not want or are unable to obtain such health assessment for their child shall be accepted by the school in lieu of the certificate.
